University Legal Services Consultation Request Form

Due to program constraints, ULS currently only offers IMMIGRATION related representation and assistance for CURRENT CSN and UNLV students, family members in their households, and their adult children and step-children; and for CSN and UNLV faculty and staff members, if their household income is less than $80,000.

NOTICE: Consultations are over the phone and are offered in 30-minute increments. Consultations are scheduled for the fourth Wednesday of the month. Consultation appointments are limited, and this form will close once all appointments are filled. New appointments will open on the first of the next month.

Contact Information for the Person Who Needs Immigration Assistance

Proof of UNLV or CSN enrollment for the current semester is required for all ULS services. This form is available at:

  1. CSN STUDENTS: GoCSN, then under the MyCSN tile, click Academics, and then “Request Enrollment Verification.”
  2. UNLV STUDENTS: MyUNLV, then under the Academic Records tile, and then on the Enrollment Verification menu item.

Qualifying UNLV and CSN faculty and staff members must not earn more than $80,000 to be eligible for ULS services. Proof of income eligibility includes the most recent tax year’s W-2 form(s), tax return, tax transcript, or the last three months of pay advice/pay stubs.
